Diagnosis probable

Presentation

Abdominal distention and discomfort two weeks after a cesarean section.

Patient Data

Age: 35 years
Gender: Female

Irregular wall thickening of the gallbladder with a heterogeneous enhancing gallbladder fundus mass lesion is noted that measures about 46*50*58 mm in its maximum diameters with an ill definition and infiltration of gallbladder fossa at the fundal region associated with multiple delayed enhancing liver metastatic mass lesions at adjacent segments (IVa and V) up to 20*27 mm.

Porta hepatis metastatic lymphadenopathy up to 14*23 mm also is seen.

Regarding above imaging findings, gallbladder carcinoma stage IIIB is the primary differential diagnosis.

Case Discussion

Our patient case was an advanced case of gallbladder carcinoma of stage III B (T3N1): 

  • T3: tumor perforates the serosa (visceral peritoneum) and directly invades the liver
  • N1: metastases to nodes along the cystic duct, common bile duct, hepatic artery or portal vein (porta hepatitis region)
