Gallbladder duplication

Presentation

2 month history of right upper quadrant pain. Abdominal ultrasound showed a cystic lesion at the hepatic hilum.

Patient Data

Age: 25 years
Gender: Female
MRI

The gallbladder appears normal in size with thin wall, no gallstones seen. Its cystic duct is well-visualized.

Above the gallbladder there is another small rounded cystic structure containing sludge (hypointense on T2), communicating with the adjacent common hepatic duct through another separate cystic duct, well-visualized on cholangio-3D-MRCP.

Case Discussion

MRCP should be performed for any suspicion of gallbladder duplication, especially if cholecystectomy is being considered, in order to inform the surgeon of the anatomical variation of the biliary system.
