Gallbladder empyema

Presentation

Right upper quadrant pain with tenderness and fever.

Patient Data

Age: 55 years
Gender: Female

Distended gallbladder with diffusely thickened and enhanced wall, and high-attenuating content (echogenic on ultrasound, not shown) with perivesicular fat stranding. Impacted gallstone (18 mm) in the gallbladder neck with no compression of the common hepatic duct (CHD) or dilatation of the IHBD. Normal CBD.

A small simple left renal cyst is noted.

Case Discussion

The clinical presentation and CT features are most consistent with a gallbladder empyema that was confirmed by percutaneous drainage.
