Gallstone ileus without pneumobilia

Presentation

Acute abdominal pain.

Patient Data

Age: 65
Gender: Male

Small bowel obstruction secondary to large obstructing gallbladder stone. 

Case Discussion

This is a case of gallstone ileus where unusually there is no pneumobilia or cholecystic gas present. The CT was initially reported as small bowel obstruction seceondary to intussusception, however a 3.3cm obstructing gallstone (with internal gas) was found at surgery.
