Ganglion cyst of the proximal tibiofibular joint

Presentation

Right knee pain for a few weeks. Anterolateral knee small bulge with increased pain for a few days. No trauma/ prior knee surgery

Patient Data

Age: 40 years
Gender: Female

The clinically visible bulge in the anterolateral infrapatellar region is a thin-walled, lobulated, anechoic, non-compressible cyst in the subcutaneous plane with adjacent echogenic subcutaneous fat. The neck of the cyst extends to the anterior side of the proximal tibio-fibular joint. There is also an elongated caudal extension of the cyst from the proximal tibiofibular joint in the leg muscle which is extensor digitorum longus.

The common peroneal nerve is away from the cyst and is not compressed by the lesion.

Normal quadriceps tendon, patellar tendon, Hoffa's fat pad, medial and lateral collateral ligaments, iliotibial band, distal biceps femoris tendon. No suprapatellar effusion. No parameniscal cyst on either side. Marginal medial and lateral osteophytes from the femur and tibia are on the medial and lateral sides.

Case Discussion

The case shows a proximal tibiofibular joint ganglion cyst with subcutaneous and intramuscular extension without peroneal nerve compression/ intraneural extension.
