Non-contrast abdominal CT scan reveals a thickened appendix with abundant gas inside and excessive surrounding fat stranding. A small mucosal defect was noticed with a few small extraluminal air bubble feature consistent with perforated gangrenous appendicitis.
The cecum is mildly thickened in resounding to appendicular inflammatory changes.
Case Discussion
A gangrenous perforated appendix was surgically proved. An analgesic drug had masked the patient's symptoms.
