Presentation

Dyspareunia.

Patient Data

Age: 35 years

A multiseptated cystic lesion located within and expanding the upper vagina, above the inferior aspect of the pubic symphysis. Although one of the smaller anteroinferior right cysts is hemorrhagic (T1 hyperintense), but the majority of cystic contents has simple fluid characteristics.

Note small retention cysts within the cervix (Nabothian cysts).

Case Discussion

Gartner duct cysts are typically located above the inferior aspect of the pubic symphysis, within the walls of the vagina. The clinical presentation is variable according to the size of the cyst from incidental finding to dyspareunia or urinary symptoms.

In this case, both kidneys were normal on ultrasound (not shown), excluding the possibility of associated urinary system anomalies.
