Presentation

4-week onset of left foot pain and swelling following a cut from a door.

Patient Data

Age: 55 years
Gender: Male
x-ray

Diffuse soft tissue air involving entire foot and part of the ankle representing extensive deep tissue infection. Evaluation of bone is limited secondary to overlying shadow of the soft tissue air. Age indeterminate fracture in the head of the second metatarsal bone. Significant soft tissue swelling.

IMPRESSION: Extensive soft tissue air in the forefoot and in the ankle extending to distal leg representing extensive severe deep tissue infection.

Case Discussion

This represents a case of extensive gangrene of the foot.

Medical history includes long-standing diabetes and hypertension.

The patient was managed surgically with immediate below-the-knee amputation and antibiotics thereafter.
