Remarkable and mass-like gastric wall thickening (axial thickness up to 22 mm) is seen in the gastric antrum, most prominently in the posterior wall and bulging into the adjacent perigastric fat. Multiple significant lymph nodes SAD up to 25 mm in the related perigastric fat and gastro-hepatic ligament fat are seen. No significant para-aortic lymph nodes and no evidence of solid organ metastasis is seen.

The case illustrates the contrast-enhanced MDCT features of the pathology-proven gastric antrum adenocarcinoma with metastatic regional lymph nodes.
