Gastric bezoar

Case contributed by Khalid Alhusseiny
Diagnosis almost certain

Presentation

Repeated attacks of vomiting. History of gastric balloon insertion.

Patient Data

Age: 70 years
Gender: Male
x-ray

Gastric balloon is seen at the left upper abdominal quadrant associated with large mottled density superior to it and relative distention of the gastric cavity.

Case Discussion

Patients with bariatric intragastric balloon therapy may present with nausea and vomiting as a side effect of the procedure itself due to the distention effect of the balloon on the stomach and sense of early satiety or may present with vomiting secondary to gastric outlet obstruction or small bowel obstruction in case of balloon migration.

In this case, the mottled opacity superior to the balloon is in favor of bezoar.
