A newborn with onset of nonbilious vomiting and a palpable mass

Abdominal plain radiography shows a large soft tissue circumferential mass displacing the bowel loops.

Modality: Ultrasound

Ultrasound reveals a cystic mass with the double wall sign (inner echogenic layer and an outer hypoechoic layer)

Modality: MRI

MRI T2-weighted sequence demonstrates a cystic mass with close contact with the gastric pylorus

Modality: Photo

Photomicrograph shows histopathologic features that are characteristics of a gastrointestinal duplication: an inner smooth muscle layer and an outer atrophic gastric mucosal lining.
