This image shows spontaneous gastroesophageal reflux of barium contrast into the mid and distal esophagus, reaching as high as the level of the aortic arch. There is also a tiny hiatal hernia, which is likely contributing to the reflux.
In addition, the patient was acutely symptomatic, as can be seen by his left hand clasped over his chest. He was feeling an acute attack of "heartburn" when this reflux occurred!
