MRI demonstrating multiple giant arachnoid granulations in the posterior fossa.

Case Discussion

Incidental giant arachnoid granulations, appearing as osteolytic, sharply circumscribed CSF-containing intradiploic cystic spaces with smooth bone remodeling.

They may fill the lumen of the dural sinus and can cause symptoms due to sinus obstruction leading to venous hypertension. They should not be misdiagnosed as extra-axial lesions or dural venous sinus thrombosis.
