Case contributed by Bahman Rasuli
Diagnosis almost certain

Presentation

Swelling and tenderness at the level of the middle finger middle phalanx.

Patient Data

Age: 40 years
Gender: Female

A 13 x 15 x 20 mm well-defined lobulated soft tissue lesion is abutting the ventrolateral aspect of the middle finger flexor tendon sheaths with wrap around the tendon causing the focal contour bulge at the level middle phalanx.

It elicits persistent low signals at all pulse sequences and moderate contrast enhancement.

Case Discussion

Tenosynovial giant cell tumors are usually benign lesions that arise from the tendon sheath. On imaging, these lesions are commonly demonstrated as localized, solitary, subcutaneous soft tissue nodules, with low T1 and T2 signal and moderate enhancement.
