The patient presented with cough and chest pain. Previously medically free patient.
Patient Data
There is prominence of the left mediastinum, at the level of the 3rd mogul of the heart, mimicking a left mediastinal mass.
There is a giant left atrial appendage measures about 3 x 2 x 6.5 cm, however it shows homogenous enhancement without definitive thrombosis. The left atrium and pulmonary veins showed normal calibre and opacification without evidence of thrombosis or enlargement.

The majority of patients with giant left atrial appendages are asymptomatic. The foremost common clinical presentations include supraventricular tachycardia and systemic embolisation 1.
Surgery is usually recommended even in asymptomatic patients to forestall possible complications of atrial fibrillation and thromboembolism 2.
