Patient presented with worsening right eye visual decline - reduced from normal vision to only being able to differentiate light and dark shapes.

Right paraopthalmic giant aneurysm measuring 27 x 24 mm. Approximately one-fourth of aneurysm appears thromobosed, with thrombus extending down to the neck. There is displacement of the anterior ventricular horns. Remainder of brain appears normal.

Rotational 3D DSA demonstrating giant right paraopthalmic aneurysm. The aneurysm is causing marked mass effect and displacement of cerebral arteries. The aneurysm neck is not particularly wide.

Case Discussion

Flow diverting stent insertion by a neuro-interventional radiologist was the definitive management for this giant cerebral aneurysm.
