Presentation

Stab wound to the right hemithorax. Crepitus of the chest upon physical examination.

Patient Data

Age: 25 years
Gender: Male

Radiograph revealed extensive subcutaneous emphysema over the right chest wall. The fibers of the pectoralis major muscle are noticeable due to the free gas in the tissue, giving it the appearance of a ginkgo leaf (ginkgo leaf sign).

There is a right pneumothorax that is marked by the visible right pleural edge.

The mediastinum is not shifted (no evidence of tension).

The ginkgo leaf sign is sometimes observed in patients with extensive subcutaneous emphysema. It is characterized by the visualization of the fibers of the pectoralis major muscle.
