Pain and joint block after anterior dislocation.

Patient Data

Age: 43
Gender: Male

Shoulder MR arthrography

Presence of loose body of chondral fragment (1 centimeter) in the lower articular recess. In axial and coronal sequences the cartilage detachment is visible in the subequatorial region, under the bare area, at 6 o'clock. There is Hill Sachs lesion to the back external portion oh the humeral head. The tendons of the rotator cuff and the long head of the biceps are normal. It is recognizable minimal detachment of the antero inferior labrum (6 o'clock) with stripped but intact periosteum (Perthes lesion).

Case Discussion

MR arthrogaphy, is commonly carried out in patients with a history of shoulder instability, usually post traumatic. In these patients, glenoid chondral defects are not infrequently found, often in association with other abnormalities such as Hill Sachs lesions and labral injury.
