Presentation
Headaches.
Patient Data
A very large mass arising from the left frontal lobe and crossing the corpus callosum is present with central areas of hemorrhage. Unfortunately the patient was unable to tolerate the whole scan (note the movement degraded T1 sequence) and no contrast was thus administered.
Case Discussion
This case illustrates typical non-contrast appearances of a WHO grade IV glioma, in this case a glioblastoma arising from an oligoastrocytoma.
Note: In a case of a glioblastoma arising from a pre-existing lower grade tumor, it would likely be IDH mutated. This case, however, predates the recent (2016) revision WHO classification of CNS tumors and IDH status is not available. As such, this tumor would now be classified as a glioblastoma NOS.
