Presented with 3 weeks of worsening loss of balance and falls.

Patient Data

Age: 65
Gender: Female

Large tumor centered in the body of the corpus callosum with heterogenous and multifocal areas of enhancement and imaging appearances typical for glioblastoma.

Case Discussion

Resection of the tumor was performed:

MICROSCOPIC DESCRIPTION: Sections of the brain show a moderately hypercellular tumor composed of astrocytes with angulated hyperchromatic nuclei. There are focal areas of pallisaded tumor necrosis and microvascular proliferation. Scattered mitoses are seen. 

Immunohistochemical stains:

  • GFAP - Positive
  • Nestin - Positive (high)
  • NogoA - Negative 
  • DH-1 R132H -  Negative (not mutated)
  • ATRX - Positive (not mutated)
  • MGMT - Negative
  • p53 - Positive
  • p16 CDKN2A - Negative
  • Topoisomerase labeling index: 25%

DIAGNOSIS: GBM, IDH-1 wild-typeWHO grade IV.
