Penetrating right eye injury. Presents with fixed dilated R) pupil and proptosis

Multiple right orbital foreign bodies with proptosis and globe elongation.

Multiple linear foreign bodies/fragments within the right orbit of varying size and location. These are located: (1) right retro-orbital space deep to superior rectus; (2) right lateral margin of the optic nerve orbital insertion; (3) inferolateral margin of the right globe adjacent to the orbital floor and another (4) at the inferior margin of this fragment; and (5) inferior orbital margin within the subcutaneous tissues.

Case Discussion

Proptosis of the right globe with elongation and extensive periorbital and retro-orbital stranding is in keeping with globe rupture, particularly in the setting of trauma.

Learning points:

  • penetrating eye injury is a common cause of globe rupture
  • CT findings suggestive of globe rupture include proptosis and globe elongation
  • other CT findings observed in globe rupture include: collapsed globe (characteristic "mushroom" appearance), enlarged anterior chamber, and thick posterior sclera
