Glomangioma - finger tip

Diagnosis almost certain

Presentation

Pain involving left little fingertip on the ulnar-dorsal side for about 1 year. Local tenderness is present without temperature hypersensitivity.

Patient Data

Age: 55 years
Gender: Male
ultrasound

There is a well-defined subcutaneous nodule (about 3 x 3 x 1.6 mm) near the ulnar edge of the nail plate. The lesion is hypoechoic with hypervascularity. The underlying distal phalanx cortex shows focal depression without erosion. Comparison with the radial side of the same finger and with the contralateral finger was done which revealed no abnormality.

1st photo shows the skin marking at the lesion. 2nd photo shows the lesion which is not under the nailbed. 3rd photo shows an excised lesion placed on the nail plate.

Case Discussion

The patient presented with localized pain in the left little fingertip. Ultrasound showed a small hypervascular lesion that is not in a subungual location. Ultrasound findings and the clinical features favored a glomangioma. Surgical excision was done and the histopathology confirmed glomangioma.
