Presentation
Right gluteal mass of 2 years duration, now the mass is slightly painful.
Patient Data
Right gluteal subcutaneous fairly defined soft tissue mass lesion. It elicits low signal on T1 WI, high signal on T2 & STIR WI. It shows intense contrast enhancement. Fibrotic scars are seen adjacent to the mass.
Multiple interstitial uterine fibroids.
Pathology after excision shows low-grade myxoid liposarcoma.
Case Discussion
Myxoid liposarcoma is the second most common subtype of liposarcoma.
Myxoid liposarcomas usually occur in the intermuscular fascial planes or deep-seated areas.
Myxoid liposarcoma that contains abundant water can mimic a cystic lesion, producing diagnostic confusion.
Myxoid liposarcomas exhibit low signal intensity on nonenhanced T1-weighted images and high signal intensity on T2-weighted images.
If myxoid liposarcomas consist of a myxoid matrix as the predominant component these histologic characteristics account for the low signal intensity on T1. In contrast, lipomas and well-differentiated liposarcomas typically show high signal intensity secondary to the relatively high fat content.
