Goiter - gigantic

Case contributed by Dr Ian Bickle

Female with long standing neck mass, only self declared to medical facility. Islamic society with hijab, so socially neck covered.

Patient Data

Age: 55
Gender: Female

Localized from CT study ( analagous to a plain film )

Massive anterior neck mass with some calcific components extending from the submandibular region to the root of the neck.

Huge heterogenous thyroid mass with some areas of dense focal calcification.

The mass extends from the mandibular angle to the retrosternal space on the left.

Trachea displaced to the right and anteriorly, but only mildly compressed.

Case Discussion

Thyroid goitres are common, however one of this size is rarer.

The enourmity of this goiter was in part due to the patient presenting late, having been able to 'hide' it under her hijab, wore for cultural and religious purposes.

The goiter was histologically benign on FNAC.
