Presentation

Post surgical thoracoabdominal pain.

Patient Data

Age: 75 years
Gender: Male

In the imaged portions of the upper abdomen, a 210 x 150 x 140 mm thick-walled collection containing gas bubbles and a curvilinear metallic density structure is noted at the right subdiaphragmatic region most consistent with Gossypiboma. The right hemidiaphragm is elevated and subsegmental collapse is noted at the right lung base.

Pneumobilia is noted (biliodigestive anastomosis?).
A few small mediastinal lymphadenopathies are seen with maximum SAD of 11 mm.
Cardiomegaly is present and pulmonary artery trunk is dilated up to 35 mm. In addition, atherosclerotic changes are seen in the aortic arch.
Degenerative changes as osteophytosis are seen in the thoracic spine.
