The patient underwent a right-sided nephrectomy. Two weeks later he presented with lower abdominal pain and tenderness.

The abdomen sonogram shows lace-like curvilinear densities projecting over the sacrum and right sacral ala.
A 60 x 70 mm sharply defined rounded mass with high-density serpiginous material within it was noted at the right side of the pelvis.
The right kidney is not seen.
The prostate is enlarged.
Thoracolumbar spine degenerative changes are seen.
Subcutaneous emphysema along with the abdominal wall on the right side is seen.
