Well-encapsulated left subphrenic ovoid hypodense structure containing air-bubbles with peripheral enhancement. A mass effect is noted on the spleen with mild reactive left-sided pleural effusion.

Thickening of the left anterior renal and lateroconal fasciae with fat stranding of the anterior pararenal space as well as a small adjacent fluid collection.

No abnormal thickening at the colo-colic anastomosis.

A small accessory spleen is noted.

The rest of the CT scan exam is unremarkable.

Case Discussion

The CT feature in a patient with a history of surgery is most consistent with a gossypiboma which was confirmed at surgery (infected gossypiboma).
