Granulomatosis with polyangiitis - pulmonary manifestations

Diagnosis almost certain

Presentation

Past history of middle lobectomy for Wegener granulomatosis four years ago. Presented with hemoptysis.

Patient Data

Age: 30 years
Gender: Female

There are multiple irregularly margined nodules randomly distributed throughout the lungs of variable size ranging from 8 to 28 mm with surrounding ground-glass opacification (surrounding hemorrhage).

No tracheobronchial wall thickening.

No mediastinal or hilar lymphadenopathy.

No pleural effusion.

Case Discussion

CT appearances of multiple and bilateral nodules randomly distributed throughout both lungs with surrounding ground-glass opacities "halo sign" in a patient with a past history of Wegener granulomatosis is highly suggestive of relapse.

The patient had CT guided-biopsy with histopathological analysis which confirmed the diagnosis of a Wegener granulomatosis.
