Hyperthyroidism for investigation.

Patient Data

Age: 40 years
Gender: Female

Pertechnetate thyroid scan

Nuclear medicine

There is homoegenous intense uptake of tracer by the thyroid gland. The thyroid appears enlarged. Uptake in tissue above the isthmus likely reflects a pyramidal lobe. No dominant hot or cold thyroid nodules are identified.

Uptake in the background soft tissues is suppressed, quantified by the markedly elevated neck to thigh ratio (normal <6).

 

Case Discussion

The classic appearance of Graves disease on pertechnetate thyroid scintigraphy is markedly increased tracer accumulation in an enlarged gland, with corresponding reduction of physiological activity in the background tissues.
