Presentation

Pronounced bilateral proptosis, preseptal swelling, and a history of Graves disease.

Patient Data

Age: 35 years
Gender: Male
ct

Enlarged lacrimal glands with prolapse, bilateral proptosis, large orbital fat, and large width extraocular muscles containing hypodense foci with preseptal swelling are seen. Temporal deviation of the right eye is also seen.
Optic nerves are low-width.

Case Discussion

The case illustrates the typical non-contrast MDCT features of Graves ophthalmopathy or thyroid orbitopathy. The most common extraocular muscles which are involved in the disease are the inferior rectus muscle, medial rectus muscle, and superior rectus muscle respectively and involvement of all extraocular muscles is uncommon and is usually seen in long-standing disease.
