Presentation
Fall on the left shoulder
Patient Data
Avulsion fracture of the greater tuberosity at the site of supraspinatus insertion.
Fracture with a cortical avulsion of the superior facet of the left greater tuberosity at the site of the supraspinatus tendon insertion with underlying bone marrow edema.
The supraspinatus tendon shows intrasubstance increased signal (tendinopathy) with a small partial-thickness articular surface tear. Decreased subacromial space with thickening of the coracoacromial ligament which may indicate background of subacromial impingement).
Increased signal with mild thickening of subscapularis and infraspinatus tendons related to tendinopathy with associated intraarticular and subacromial/subdeltoid effusion as well as around the long head of the biceps tendon.
Case Discussion
MRI features are of humeral greater tuberosity avulsion fracture with a partial supraspinatus tendon tear on a background of subacromial impingement. Infraspinatus/subscapularis tendinopathy.
