The vagina is distended (11.9 x 7.3 x 7.2 cm), and filled with fluid contents displaying T1 and T2 hyperintense signal. The uterus is superiorly and anteriorly displaced by the distended blood filled vagina. The urinary bladder is displaced anteriorly.  

Dilated tortuous right fallopian tube with the same internal content MRI signals of blood content, reflecting associated right hematosalpinx. 

Case Discussion

Hematocolpos refers to a distended vagina by retained menstrual blood after puberty, commonly due to imperforate hymen. It may also distend the uterine cavity (hematometrocolpos) and can be associated with hematosalpinx. Other causes can be transverse vaginal septum, vaginal atresia, or stenosis.
