Diagnosis almost certain

Presentation

Dysuria, cyclic lower abdomen discomfort, amenorrhea, and increasing abdominal distention.

Patient Data

Age: 13 years
Gender: Female
ultrasound

Lower endometrial and endovaginal fluid accumulation is noted, with low-level internal echoes. Color Doppler imaging reveals no increased vascularity.

The left ovary contains a well-defined thin-walled, anechoic simple cyst with posterior acoustic enhancement.

The right ovary appears normal.

Case Discussion

The findings are consistent with hematometrocolpos. It occurs when menstrual blood accumulates in the uterus and vagina due to obstruction of the cervical or vaginal outflow systems. Possible causes include congenital abnormalities or acquired blockages like imperforate hymen or vaginal septa.
