Presentation
1-2 weeks of lower abdominal/pelvic pain, some associated vaginal bleeding and discharge, mild fever, and recent abdominal trauma sustained from a fall while skiing.
Patient Data
Transabdominal and transvaginal pelvic ultrasound was performed with the patient's consent. A normal appearing anteverted, anti-flexed uterus is present. Endometrial stripe (trilaminar) measures 5mm. The right ovary has a normal appearance. The left ovary is intimately related to an exophytic complex cystic mass measuring 3.6 x x3 x 4.1cm. It has an US appearance most in keeping with an hemorrhagic cyst. A serum beta HCG was performed to exclude an ectopic pregnancy (normal). No other adnexal mass or free pelvic fluid identified.
Case Discussion
A likely hemorrhagic left ovarian cyst. A repeat US to exclude underlying neoplasm/endometrial cyst is recommended in 6 weeks time.
