Presentation

Progressive dyspnea and leg swelling, newly developed atrial fibrillation on EKG. CHF?

Patient Data

Age: 55 years
Gender: Male
x-ray

Marked cardiomegaly, broadened hilar vessels, Increased bilateral, hazy interstitial markings with a basal predominance, however no unequivocal Kerley B lines. 

4 years earlier exam

Prior negative exam for comparison. 

Case Discussion

Typical imaging findings of congestive heart failure without frank pulmonary edema. Subsequent echocardiography showed diffuse ventricular and atrial dilation, significantly reduced left ventricular function and hypokinesis, with septal akinesis.
