Presentation

Three-week history of headaches.

Patient Data

Age: 20 years
Gender: Male

A solid and cystic mass is centered within the inferior fourth ventricle extending to the foramen magnum. The solid portion of the mass enhances densely and the multiseptated cystic portion of the mass is located inferiorly. The mass compresses the dorsal cord at the cervical medullary junction with mild adjacent cord edema and a probable syrinx is seen in the upper cervical cord. The mass obstructs the fourth ventricle resulting in mild hydrocephalus but no transependymal edema is seen. The obstructing solid and cystic tumor within the inferior fourth ventricle compresses the dorsal surface of the cervicomedullary cord resulting in cord edema and there is a tiny syrinx extending to the level of C3-C4.
