Hematoma following central line placement

Presentation

Liver failure, post-op liver transplant, line placement.

Patient Data

Age: 35 years
Gender: Male

After line placement

x-ray

Placement right IJ central line, with tip at the cavoatrial junction. No pneumothorax. Lungs clear. New right paratracheal soft tissue density displacing the trachea to the left.

Earlier the same day

x-ray

Normal, including right paratracheal stripe.

Case Discussion

Portable chest x-rays are often obtained to determine if lines and tubes are appropriately positioned and to exclude complications. Ideally, central line tips should be in the superior vena cava or at the cavoatrial junction. Perhaps the most common complication of central line placement is pneumothorax. There was no pneumothorax in this case, but the new right paratracheal soft tissue density was suggestive of a hematoma.
