Hemispherical spondylosclerosis (MRI)

Chronic backaches without sciatica.

Patient Data

Age: 50
Gender: Female

T1- and (predominantly) T2- hypointense lesion adjacent to the ground plate of the 2nd lumbar vertebral body with a dome-like upper border. Note the presence of degenerative disc disease (DDD) at L2/3, compared with the segments above and below.

Case Discussion

This polyaetiologic, but mostly degenerative lesion must be differentiated from neoplastic or infectious conditions. It is probably identical to type III vertebral end plate changes in DDD, as described by Modic.
