Presentation
Incidentally discovered lesion on ultrasound
Patient Data
A large well-defined globular shaped exophytic mass with lobulated outlines is seen originating from the right lobe & bulging from the superior surface of the liver elevating the right hemidiaphragm with a subtle displacement of the hepatic flexure of the colon, it measures about 15x9 in maximal CC & transverse diameters respectively. In the precontrast scan, it is seen as relatively hypodense with multiple hypoattenuating areas inside, following contrast administration the mass displays nodular peripheral enhancement in the arterial & porto-venous phases (the enhancement density follows the aortic attenuation in all phases), in the delayed phase the enhancement is similar to hepatic attenuation & increased progressively to involve a large peripheral area of the mass. In addition, another smaller lesion with similar features is seen in segment IV.
Case Discussion
A 40-year-old female with no significant past medical history presented with a large hepatic mass incidentally discovered during an abdominal ultrasound. Further evaluation with triphasic CT confirmed the mass to have characteristics picture of a giant hemangioma
