Hepatic hemangiomas - focal nodular hyperplasia

Case contributed by Ammar Haouimi
Diagnosis almost certain

Presentation

Known for hepatic lesions for a few years.

Patient Data

Age: 45 years
Gender: Female

There are multiple well-defined hepatic lesions of various sizes, disseminated in both lobes, the largest lesion is located in the left lobe measuring (12 x 10 x 9 cm). They display a low signal on T1, high signal on T2/T2 fat sat with a discontinuous nodular peripheral enhancement on arterial phase, more centripetal fill-in on portal venous phase with almost complete fill-in on delayed sequences except for the largest lesion (giant hemangioma).

Another well-defined rounded lesion is noted in segment VI (2.5 cm in diametre), isointense to the liver on all sequences with early intense enhancement on arterial phase and isointense to the liver on portal venous phase suggestive of focal nodular hyperplasia.

Case Discussion

MRI features of multiple hepatic hemangiomas, the largest lesion (size>4 cm) are called giant hemangioma with associated focal nodular hyperplasia.

Hepatic hemangiomas are the most common associated benign lesions with focal nodular hyperplasia.
