Abdominal distension.
Patient Data
A huge mass replaced almost the entire right lobe of the liver and parts of the left lobe as well (segments I, IVa, IVb, V, VI, VII, VIII) measuring about 18x 13x 13 cm.
The lesion demonstrates multifocal avid early arterial enhancement with relative filling in the portal-venous phase and more filling in the delayed phase. The enhancement pattern is similar to the aorta in all phases.
There are about 10 other smaller lesions of similar features in segments IVb and V of the liver. The largest of these lesions measures about 1.8 x 1.6 cm.
The segments II and III of the liver are spared.
The vessels are mainly displaced by the lesion rather than significant encasement.
No extension beyond the liver margins is noted.
Case Discussion
Considering the enhancment pattern the features are more likely of hepatic haemangiomatosis rather than multifocal hepatoblastomas or infantile hemangioendothelioma which usually occurs in younger age group.
