Hepatocellular carcinoma with tumor thrombus

Diagnosis almost certain

Presentation

History of liver mass for evaluation. Hepatitis screening in our institute reveals that the patient is positive for hepatitis C.

Patient Data

Age: 80 years
Gender: Male

Quadraphasic liver scan was performed which demonstrates a large mass infiltrate segment 7 and 8 of the liver, enhancing during the arterial phase and washout in portovenous phase consistent with hepatocellular carcinoma. The liver otherwise is unremarkable. No features of cirrhosis or portal hypertension are noted. Incidental gallbladder stones are seen.

Followup - 2 months later

On follow up study after 2 months the patient develops tumor thrombus involving the middle hepatic vein extending into the IVC.
