Hepatocerebral degeneration associated with heterozygous hemochromatosis

Diagnosis probable

Presentation

Late onset psychosis

Patient Data

Age: 55
Gender: Male

Multiplanar, multisequence imaging has been obtained through the brain including whole brain coronal sequences on the 1.5T scanner.

There are no intra- or extra-axial mass lesions, acute hemorrhages or collections. No acute or established infarcts. There is a mild extent of bi-hemispheric chronic white matter ischemic change. On T1WI, the globus pallidus bilaterally demonstrate high signal on T1WI with associated mild suscepability blooming - likely related to hepatic failure secondary to the known hemochromatosis.  Mild, generalized cerebral involution, mildly exaggerated for age.  No regions of abnormal parenchymal or meningeal enhancement.

Although this study has not been targetted to the mesial temporal lobes, no overt hippocampal abnormality demonstrated. No regions of cortical developmental malformation.

Conclusion:

No intra- or extra-axial mass lesions, acute hemorrhages or collections. No acute infarcts. High signal on T1WI imaging within bilateral globus pallidi likely relate to hepatic failure secondary to the known hemochromatosis.
