Diagnosis probable

Presentation

Progressive lower limb weakness. Her two siblings have the same problem.

Patient Data

Age: 20 years
Gender: Female
mri

Dorsal spinal cord mild atrophy evident by the prominence of CSF spaces.

No focal lesion.

The conus medullaris terminates at a normal level.

The bony spinal canal is normal.

No evidence of disc desiccation or disc bulge.

No spinal canal stenosis.

Case Discussion

The patient was clinically suspected for hereditary spastic paraplegia (HSP). Spinal cord atrophy is an imaging feature seen in this entity, though it is not specific for HSP.
