Hernia of the broad ligament of the uterus

Sudden onset abdominal pain, nausea and vomiting. History of cesarean section and appendectomy.

Patient Data

Age: 40 years
Gender: Female

Dilated loops of small bowel intestine with transition point located in the right lower quadrant. Abnormal position of the uterus in close contact with the anterior abdominal wall. A loop of the small bowel, forming a “C” shape, is trapped in the space created by the anterior wall of the uterus, the right tube, and the dome of the bladder. The distal bowel is decompressed.

Case Discussion

Laparotomy revealed pelvic adhesions between the uterus and the anterior abdominal wall with a small opening in the right broad ligament of the uterus through which a distal ileal segment entered. A resection of the pelvic adhesions was performed. The incarcerated ileum was released and preserved because there were no ischemic changes.
