T2 and FLAIR hyperintensities with asymmetrical distribution of bilateral temporal lobes, inferolateral frontal lobes, and bilateral insula ribbon with associated edema and restricted diffusion.

Case Discussion

MRI features are classic of herpes simplex encephalitis

  • asymmetric involvement of the temporal lobes
  • asymmetric involvement of the insular cortices
  • gyrus rectus and cingulate gyrus involvement

There is typical diffusion restriction in the affected areas.
