Retrocardiac opacity with air-fluid level.

Herniated stomach and pancreatic tail within the lower thorax. Also, there is a widening of the esophageal hiatus.

Typical case of an incidental hiatus hernia. Note that here not only the stomach but also the pancreatic tail herniated into the thorax.
