Presentation
Seizures with loss of consciousness when falling asleep. Clonic cramps in the extremities, with paralysis of the right hand (Todd's paralysis), duration of about 2 minutes. Electroencephalogram: epileptiform patterns in the left frontotemporal region.

There is an abnormal vertical orientation of the hippocampal body and collateral sulcus.
Case Discussion
An asymmetric hippocampus should be differentiated from structural variational of the normal medial temporal lobes, however, given the presence of clinical symptoms this is most likely hippocampal malrotation.
