Presentation

Bilateral hand and forearm muscle wasting and atrophy.

Patient Data

Age: 20 years
Gender: Male

The spinal cord is significantly thinned at the C5-C6 level. Has an increased signal on T2 - manifestations of myelomalacia. In the neutral position, the spinal canal is not narrowed, the nerve foramina are without features.

In the flexion position (up to 40-45 degrees), sagittal and axial T2 tomograms were obtained. There was an anterior displacement of the posterior dura at the C5-C7 level due to the expansion of the epidural venous plexus.

Case Discussion

Hirayama's disease is a form of muscular dystrophy observed in the 2nd or 3rd decade of life.

This disease is classically manifested by bilateral weakness and atrophy of the muscles of the forearm and hand.

This case shows the typical manifestations of Hirayama disease and demonstrates the need for flexion to confirm the diagnosis.
