MRI study shows a lobulated cystic lesion within Hoffa's fat pad centrally and extending intraarticular. It is likely extending into or originating from the anterior cruciate ligament with associated ligamentous thickening and signal changes. It elicits fluid signal intensity with thin internal septa.

Here is a case of Hoffa fat pad ganglion cyst, one of the cyst-like lesions around the knee. It is likely arising from or extending into the anterior cruciate ligament. One of the theories of Hoffa fat pad ganglion cyst. Pathology is mucoid degeneration, which is suspected to be the cause in our case, considering associated ACL changes
